  • 衛生教育在結合行動科技上,已具有許多不同的衛教系統平台,但針對現況分析進行初步設計之醫病溝通互動的衛教平台較鮮少,多為一般健康資訊或常見慢性等疾病,對風濕免疫科門診而言,疾病知識與衛生教育較為缺乏,鮮少具友善病人使用的平台系統。本研究旨在建置評估數位化醫病衛教平台功能整合應用,初步於風濕免疫科門診病人之醫師信任溝通、衛教疾病管理、疾病不確定感、生活品質深入暸解其門診現況。本研究為先導性研究,實驗階段一為調查性研究,實驗階段二為質性研究,以北部某醫學中心門診收案為期六個月,問卷調查151位及半結構式訪談6位病人,針對病人現況及研究者設計之數位化醫病衛教平台進行研究分析與探討。
    研究結果顯示: (ㄧ)多數病人主要具有疼痛、睡眠品質和藥物副作用等症狀被困擾,確診初期生活品質大受影響,同時也影響心理健康。此外,易受到相同疾病之病友的罹病經驗擔憂。(二)病人主要透過門診時與醫師諮詢與衛教,多數人疾病先備知識不足且具理解落差,醫院只提供傳統紙本及電子衛教單張,較低的閱讀動機及無互動諮詢影響疾病認識。(三)多數病人透過門診與醫師諮詢,及控制自身疾病,易受限於醫療院所診間狀況繁多及醫師忙碌等,在就診感受、醫病溝通上產生負面影響。(四)本研究綜合評估病人,數位化醫病溝通平台的需求及期待,提供病人疾病初次確診及尚未穩定控制時期,得以紀錄每日症狀,以利醫師瞭解病況,且依據不同疾病需求提供健康日記功能,期待有更多機會及功能與醫護進行互動,但同時也表示不希望額外造成醫護更多的工作負荷量。


    Many different health education systems for mobile technology applications, the preliminary design of a digital health education platform for doctor-patient communication and interaction is relatively rare, and most of them are for general health information or common chronic diseases. For rheumatology and immunology outpatient clinics, there’s a lack of knowledge about diseases and health education, and few patient-friendly systems are available. The purpose of this study is to construct and evaluate the functional integration application of the digital medical and disease health education platform, and to initially analyse the current outpatient situation of rheumatology outpatient clinics in terms of physician trust communication, health education disease management, disease uncertainty, and quality of life. This study is a pilot study, experimental stage 1 is a survey study, experimental stage 2 is a qualitative study, a medical center in the north of the outpatient cases for a period of six months, questionnaire survey of 151 and semi-structured interviews with 6 patients, focusing on the patient's current situation and the researcher's design of the digital medical health platform for research and analysis and exploration.
    The result of study that : (a) Most patients are disturbed by symptoms such as pain, quality of sleep, and side effects of medications, which greatly affects their quality of life in the early stages of diagnosis, as well as their psychological well-being. In addition, they are easily worried by the experience of patients with the same disease. (b) Patients mainly consult with doctors and receive health education at outpatient clinics. Most of them do not have enough prior knowledge of the disease and there’s a gap in their understanding, and hospitals only provide traditional paper and electronic health education leaflets, which affects their understanding of the disease due to their low motivation to read and the lack of interactive consultation. (c) Most patients consult with doctors through outpatient clinics and control their own diseases, but easily limited by clinic conditions and busyness of doctors in institutions, which negatively affects their feelings of consultation, communication between doctors and patients. (d) Study comprehensively evaluates needs and expectations of patients for digital medical communication platform that provides patients with the ability to record their daily symptoms when their disease is first diagnosed, not yet under stable control, can understand their illnesses, provides diary function according to the needs of different diseases, hope that there will be more opportunities and interact with healthcare.And they also expressed the desire not to create additional for professionals.
