隨著高齡化社會的出現,牙齒治療的需求與日俱增。然而患者面對缺牙治療,醫生多傾向植牙或牙橋,即侵入式治療。其中潛藏著醫療資訊不對稱的情形。本研究希望拉近醫師與患者間專業知識落差的情況下,亦即提供侵入式與非侵入式治療的情況下,了解患者在做侵入式與非侵入式治療的選擇,受到那些因素的影響。
本研究以問卷影片方式,讓受訪者了解缺牙的治療方式有三種-人工植牙、牙橋與活動式假牙,並依治療方式區分為侵入式治療(人工植牙、牙橋)與非侵入式治療(活動式假牙)。藉由計畫行為理論分析,並加入外生變數-醫療風險、費用高低與美觀方便,探討患者對治療方式的選擇模式、態度及其受影響因素。
研究結果顯示超過六成的受訪者選擇非侵入式治療,即願意先嘗試活動假牙。醫療風險是患者最為在意的因素,其次是美觀方便。在意治療費用的患者會選擇非侵入式治療,選擇侵入式治療者則不在意費用高低。患者本身的想法(態度)之於侵入式與非侵入式皆呈現高度正相關﹔對患者有影響力的人之治療經驗或看法(主觀規範)高度正相關於選擇非侵入式治療者﹔至於患者對自己選擇的信心程度(知覺行為控制)之於侵入式治療為顯著負相關,對非侵入式治療為顯著正相關。
Demand for denture treatment is increasing with the advent of an aging society. For dentulous treatment, dentists tend to suggest the invasive type such as implant or dental bridge. Such a situation is often associated with a medical information asymmetry between dentists and patients. This study aimed to delineate the principal factors that may influence patients in choosing edentulous treatment when the gap of medical expertise between dentists and patients (the difference between invasive and non-invasive treatments is well explained) is narrowed.
For subjects understanding of the three treatments – implant, dental bridge, and removable dentures by viewing a questionnaire video. Denture treatments in the video were classified into two types: invasive (implant and dental bridge) and non-invasive (removable dentures). Based on the Theory of Planned Behavior (TPB) analysis with the addition of three exogenous variables – medical risk, expense, and appearance/convenience, we investigated how patients’ perception of the various edentulous treatments may influence their decision-making process and what factors are considered critical.
The result shows that more than 60% respondents chose non-invasive over invasive treatments, implying that they are willing to try removable dentures first. From a patient’s view, medical risk is a key concerned factor, followed by appearance and convenience. Patients who care treatment costs tend to choose the non-invasive type, whereas who choose the invasive usually do not care about costs. Significantly positive correlation is found between the thoughts of the patients themselves (attitude) and the treatments. The treatment experiences or advices from the people who are important for patients (subjective norm) are significantly positive correlation with the decision made by those who choose the non-invasive treatment. Confidence level in a patient’s choice (perceived behavioral control) is notably negatively and positively correlated with the invasive and non-invasive treatments, respectively.
