  • 本研究蒐集90至94年全國營造業墜落重大災害事故421件個案,藉由分析個案之人員、作業、設備、環境、災害原因等因素並找出各種因素之間的相關性,再經由意外事故分析技術AcciMap呈現墜落災害之關鍵事件前的原因及產生的後果,從AcciMap之階層關係調查最上游的法令政策至最下游作業勞工所面臨的施工環境及設備,從政府部門至現場環境與設備的階層關係中,分析從每一階級之安全衛生管理單位對於勞工安全衛生相關法令及環境危害因素評估,分析各階層之決策與墜落災害的關係。
    為了找出墜落災害原因的流程及找出有利改善的關鍵因素,進而提供每一階層改善系統內災害發生之原因與安全考量計畫的事件順序,利用AcciMap來呈現影響墜落災害流程的決策與行為,從政府部門的法令制訂到現場作業環境之階層流程順序,除了顯示出墜落的相關因素,更能提供每一階層單位制訂墜落災害之決策。本研究將探討範圍限制於不安全的情境分析,依據作業者行為、環境與設備等風險因子作邏輯性的分析肇災流程順序,並以AcciMap發展出不同施工階段的各作業項目於特定工作地點之墜落災害不安全情境,應可簡明有效的提供現場作業管理者採取對應的墜落災害預防對策。


    This study collected 421 cases of fatal falls in the construction industry of Taiwan in 2001~2005. By analyzing the case of personnel, operations, equipment, environment, cause of disasters and other factors and to identify the reasons for the correlation between various factors, and then through accident analysis techniques “AcciMap” presented key events of fatal falls before the disaster and the consequences of the construction environment and equipment from the most upstream “AcciMap” investigation hierarchical relations Act policy to most workers are facing downstream operations, from government departments to the scene environment and equipment hierarchical relationship, the analysis from the safety and health management unit for each class of worker safety and health hazards related laws and environmental assessments, analyze the relationship between the various levels of decision-making and fall hazards.
    To find out the cause of the fall hazards and find the key factors enabling process improvements, thereby providing reasons occurring within each stratum improve system security considerations disaster plan and sequence of events, the use of “AcciMap” to render decision-making processes affecting fall hazards and behavior , from government departments to formulate laws to the class of sequential processes live operating environment, in addition to showing the fall of the relevant factors, but also provides the decision-making at every level units to formulate fall disasters. This study will examine the scope of restrictions on unsafe scenario analysis, based on risk factors such as worker behavior, environment and equipment for analysis Hajime disaster sequential logical processes, and use “AcciMap” to develop different stages of construction projects each operation insecurity situation in a particular workplace hazard of falling, it should be concise to provide effective field operations managers to take the corresponding fall disaster prevention countermeasures.
