本研究以縱橫資料(panel data)迴歸模型探討影響台灣婦產科診所經營績效之因素,並採用健保資料庫、內政部統計處及中華民國統計資訊網1998年至2012年共1,259間台灣婦產科診所的資料。本研究利用BP-LM Test、Hausman Test及Over-Identification Test of Hausman Taylor Model (HTM) 篩選適當的實證模型。
實證結果發現,在診所可控制的內部資源中,醫師數、護理人員數與健保病床數的增加將為婦產科診所帶來更多的健保點數,其結果隱含了台灣婦產科產業人力短缺和工作負荷量沉重的現況,除此之外,增加診所看診的科別總數,也能對婦產科診所經營績效的提升有所幫助。在外部政策與環境的影響方面,實施總額支付制度後,診所面臨更加劇烈的健保點數競爭,為了獲取利潤,各診所選擇增加醫療服務量以申請到更多的健保點數。而失業率與人均可支配所得對於婦產科診所的點數有負向的影響,顯示若患者的經濟狀況改善,其可能會選擇較高醫療層級的醫療機構就診。


This study constructs a panel data regression model to examine the determinants of OB/GYN clinics’ operational performance with data for 1,259 OB/GYN clinics over the period from 1998 to 2012 from Taiwan’s National Health Insurance Research Database (NHIRD) and R.O.C. National Statistics. This study employs three tests including the Breusch-Pagan Lagrange multiplier (BP-LM) test, the Hausman test and the over-identification test of the Hausman-Taylor model to choose the appropriate empirical model.
The empirical results of the fixed-effects model reveal that numbers of obstetricians, gynecologists, nurses, insured beds and divisions have significant positive effects on performance of OB/GYN clinics. This situation implies that increasing medical personnel could improve the efficiency of outpatient services and raise the points of clinics. Besides, setting up polyclinics will also earn more money than specialist clinics. Furthermore, the implementation of the GBS has a positive impact on the total points. The reason may be that in order to earn more revenue, clinics strive to obtain more points in spite of a decline in the point value. By contrast, macro indexes such as the unemployment rate and per capita disposable income have negative impacts on the total point of OB/GYN clinics.
