輕度認知障礙被視為正常老化以及失智症之間的過渡階段,因此如何延緩或預防輕微認知障礙老人退化成失智症,已是科學界致力研究的方向。目前對於輕度認知障礙的老年人,普遍的共識為採取複合認知訓練加入運動訓練的介入方式,而新型輔助科技的發展如體感運動遊戲與虛擬實境遊戲,則提供了更多元化的訓練環境與效果,因此本實驗目的為比較虛擬實境訓練與複合式訓練間在認知以及雙重任務步態表現。實驗共招募62名輕度認知障礙患者,透過前測評估後分為虛擬實境訓練組與複合式訓練組每組各31人,並在15週每週2次共30次的訓練後進行後測評估。本實驗目前完成收案者為24人,虛擬實境訓練組與複合式訓練組各12人,結果發現在認知部份虛擬實境訓練組在蒙特利爾認知評估;中文版加州語文學習測驗的第二次嘗試、30秒延遲以及20分鐘延遲;史楚普叫色45秒答對個數與總答題時間有顯著進步;在執行3公尺起走的雙重任務步態中,兩組在單一任務的平均速度以及平均步長,以及認知型雙重任務的平均步長以及步幅時間有顯著的進步。與複合式訓練相比虛擬實境訓練組在整體認知與記憶上有顯著進步,在認知雙重任務中,兩組的長度和步幅都有所改善。未來若能夠再增加訓練時間、訓練強度或更多元的訓練方式,虛擬實境訓練應該能夠達到更好的訓練效果。


Mild cognitive impairment (MCI) is regarded as a transitional stage between normal aging and dementia. Therefore, how to delay or prevent degeneration in dementia of the elderly with MCI is the direction of the scientific community. The development of new assistive technology, such as exergaming and virtual reality (VR) games, provides more diverse environment and effects. The purpose of this experiment is to compare the cognitive and dual task gait performance between VR training and multicomponent exercise training. A total of 62 patients with MCI were enrolled in the trial. After pre-test evaluation, they were divided into VR training group and control training group doing multicomponent exercise training, each group consists of 31 people. After 30 times of training, twice a week for 15 weeks, they underwent post-test evaluation. Outcome measure include cognitive function and dual task gait performance. The current study completed 24 people trials, 12 people in VR group and multicomponent exercise group, respectively. The results found in the cognitive part of the VR training group in Montreal cognitive assessment (p=0.021), Chinese version of the California language learning test (second attempt, p=0.025; 30-second delay, p=0.010; 20-minute delay, p=0.016), Stroop test (45-second answer, p=0.022; the total answer time, p=0.000) were significantly improved. In the dual-task gait of 3 meters timed up to go, VR group average speed (p=0.013) and length (p=0.018) have significant difference in the single task, also the length (p=0.001) and stride time (p=0.005) in the cognitive dual task. In the control group, average speed (p=0.002) and length (p=0.005) have significant change in the single task, also the length (p=0.000) and stride time (p=0.015) in the cognitive dual task. Compared with the control group, the VR training group has significant progress in overall cognition. In the cognitive dual task, both VR and control group have improved in length and stride time.
