  • 本研究利用panel data動態回歸模型,探討影響台灣診所經營績效之因素。本研究採用全民健康保險研究資料庫(NHIRD)、內政部統計處及中華民國統計資訊網1998年至2012年間共9,109間台灣診所的資料。本文的普通最小二乘法(OLS)、似無相關回歸(SUR)和兩階段最小二平方法(2SLS)三種模型中,以2SLS為合適的實證模型。此外,本研究還採用Unit-roots和Panel cointegration來檢查變數之間的長期均衡關係。
    透過2SLS模型的實證結果發現,老年人口和撫養比的增加對於所有診所的表現都有顯著的正面影響。這種情況意味著老年人口和撫養比的增加可以提高門診服務的效率並為診所帶來更多的健保點數。此外,診所如為老年人提供特殊照顧服務將有助於提升診所績效。而失業率與國內生產總值(GDP)對於診所的點數有負向的影響,顯示若患者的經濟狀況改善,其可能會選擇較高醫療層級的醫療機構就診。


    This study constructs a dynamic panel data regression model to examine the determinants of clinics’ operational performance with data for 9,109 clinics over the period from 1998 to 2012 from Taiwan’s National Health Insurance Research Database (NHIRD) and R.O.C. National Statistics. This study employs three models including the Ordinary Least Squares (OLS), the Seemingly Unrelated Regressions (SUR), and the Two Stage Least Squares (2SLS) to find an appropriate empirical model. In particular, the Unit-roots test and Panel Cointegration test are utilized to examine the dynamic model.
    The empirical results of the 2SLS model reveal that elderly population and dependency ratio have significant positive effects on performance of all clinics. This situation implies that increasing elderly population could improve the efficiency of outpatient services and raise the points of clinics. Besides, setting up the provision of special care for elder will improve performance. By contrast, the unemployment rate and GDP have negative impacts on the total point of all clinics.
